This great DuraVent PelletVent Pro Tee includes the clean-out adaptor.
The 4" diameter tee is 12 5/8" tall. The distance from the center of the take-off to the bottom of the clean-out adaptor is 7 1/2" for the 4" tee.
Use this tee with clean-out adaptor to start your vertical installation when the appliance has a flue collar/take-off on the back.
